The Physicians of GPAM want parents to realize the importance of annual checkups after the age of 3. A checkup is beneficial in evaluating the health, growth, and well-being of your child. Since many children need a sports form completed to participate in after school activities - We will complete a "sports form" at the time of your check up and make sure that the form can be used for any sport that your child participates in throughout the year.
Since our physicians feel that an annual checkup is so important, we are completing all routine sports forms without a fee if your child is up to date on checkups. HOWEVER, any form needed urgently (within 48 hours) will incur a $10 fee.
REMEMBER, SPORTS ARE GREAT FOR CHILDREN, HOWEVER, WE NEED TO MAKE SURE THEY ARE HEALTHY BEFORE THEY PARTICIPATE - YOUR PHYSICIAN IS THE BEST ONE TO EVALUATE YOUR CHILD.
